Visitor Request - Please submit Visitor Request form a minimum of five (5) business days prior to visitor arrival on campus. This will allow time for the departmental preparation, approval process, and securing reservations for the visitor. (e.g. parking permit, shuttle, and lodging). It’s recommended to submit this form as soon as the visitation dates are confirmed with the visitor.
Seminar/Colloquium Information Submission
Exam Printing Request
All printing requests must be submitted at least 5 FULL BUSINESS DAYS IN ADVANCE. A full week and half's notice, 7 BUSINESS DAYS, is needed for all mid-terms and final exams due to the high volume of orders.
Requests submitted after 12:00 noon will be counted as received on the following business day and do not count towards the 5 day notice.
Weekends, Holidays and the Day of the Exam are not included in the 5 FULL BUSINESS DAYS. Exams are printed at Printing and Reprographics, so please plan ahead and give yourself and the staff time to process your order. This will ensure that your printing requests are completed on time.
ANY RUSH ORDERS THAT ARE MADE (LESS THAN THE TIME WE HAVE ASKED FOR) WILL INCUR A FEE TO BE PAID BY THE PERSON SUBMITTING THE ORDER. RUSH ORDERS ARE BEST DROPPED OFF AT REPOGRAPHICS - THEY STILL REQUIRE 4 HOURS TO COMPLETE.
**A rush order fee of 15% or $15 (whichever is greater) will apply to all next day orders.
A rush fee of 25% or $25 will apply to same-day orders.** Again, the Math Department will not cover the cost of these fees.**
